italk’s Discharge Protocol:

All clients are required to have a discharge evaluation to compare performance from the initial evaluation. Our discharge protocol is as follows:

  1. The clinician notes that there have been improvements that warrant a discharge evaluation

  2. The clinician notes progress of short term and long term goals completed to the caregiver and initiates the discharge conversation

  3. Evaluation is completed by a non-familiar SLP for objective scoring purposes

  4. Client is discharged if Standardized and Dynamic assessments warrant discharge

  5. Final meeting is scheduled to provide:
    - Speech Therapy Diploma
    - Gift (goody bag for children, something else for adults)

  6. Schedule a follow-up for 3 or 6 months
    - schedule as an event on SimplePractice so that you don’t forget to email!

  • The clinician should always be the one to initiate discharge - be up to date with goal progression and make discharge goals very clear.

    When co-treating a client, the Leading Clinician should always be the one to initiate and follow up.

  • The Reception Team will schedule the evaluation with an unfamiliar SLP, after the leading SLP reaches out to the parents to talk about potential discharge and plans for an evaluation.

  • The SLP will email the client/caregiver 3 months or 6 months post-discharge to follow up - if a parent/caregiver interview needs to be scheduled, SLP works in conjunction with Reception Team
